What is compounding?

Compounding
is the art and science of preparing customized medications for patients. To cater to the many patients who have unique needs and preferences, a compounding pharmacist can formulate a customized prescription to change the strength of a commercial medication, alter its form to make it easier to ingest or simply add flavor to make it more palatable. Medication can also be formulated for unique delivery through dosage forms like a sublingual troche or lozenge, a lollipop, or a transdermal gel or cream.

Lozenges / Troches
 
Lozenges or troches are a popular means of keeping drugs in the mouth when local action is needed there. They can also be placed under the tongue and allowed to dissolve for sublingual delivery. This allows the medication to enter the bloodstream quickly and easily. Lozenges can be enhanced with natural sweeteners and pleasant tasting flavours, making them ideal for geriatric and paediatric patients.
 
Other Unique Oral Delivery Systems
 
To counter the difficult time that many patients have in taking conventional oral medicine, many medications can be formulated into a flavoured lollipop or a frozen ice block. Others can be taken as chewable forms. Infants especially benefit from alternate delivery devices such as pacifiers or baby bottles that are great for dispensing medicine easily and accurately.
 
Transdermals
 
With transdermals, the absorption of medicine occurs directly through the skin. Gels, emulsion creams as well as sprays and lip balm stick applicators are easy to use and effective. These transdermals are often prescribed for pain management and inflammation as well as to treat nausea and vomiting. Transdermals can help to reduce side effects like stomach upset or drowsiness.
 
Suspensions
 
For patients who do not like the taste of a commercially available liquid or have difficulty swallowing tablets and capsules without any readily available liquid form of their medication, compounding has an answer. Naturally sweetened, pleasant tasting oral solutions or suspensions can be administered easily and accurately.

Powder Insufflators
 
Insufflators can apply an extremely fine dry powder to the nose, throat, ears or other body cavities. They can be applied topically for wounds. By squeezing a bulb, a patient can direct a powdered medication through a nozzle for direct application. Dry powder insufflators offer rapid onset of treatment, and can minimize adverse reactions by releasing small, accurate doses. They are often used in the treatment of earaches.
 
Suppositories
 
Compounded suppositories are useful for patients who cannot take medication orally. They come in various shapes and depending on the site of administration, can be given rectally, vaginally or urethrally. As the suppository melts into the body cavity, the active ingredients pass rapidly into the bloodstream. Suppositories can be used for delayed-release medications, hormone replacement therapy, or to treat local conditions such as nausea, haemorrhoids, infections or inflammation.
